At TRU PACE, you will receive coordinated medical care from a team of professionals who specialize in aging and chronic conditions. Our medical team of doctors, nurse practitioners, and registered nurses work with therapists, dietitians, social workers, and home care nurses to create a customized plan. It is our goal to ensure you have optimal health, safety, and overall well-being.
The heart of the TRU PACE program is the Interdisciplinary Team (IDT), a team of specialists that communicate regularly to determine your plan of care along with you and your loved ones. When you enroll in TRU PACE, you will meet with all of the following professionals:
- Primary Care Physician
- Center Director
- Clinic Nurse
- Social Worker
- Recreation Therapist
- Home Care Nurse
- Dietitian
- Home Health Aide
- Occupational Therapist
- Physical Therapist
- Transportation Coordinator
TRU PACE participants must receive all services including primary care and specialist physician services (other than emergency services) from TRU PACE or an entity authorized by TRU PACE.
