Who We Are

Living at home: Your goal, our priority

TRU PACE is for older adults who meet the guidelines for enrollment both functionally and financially. TRU PACE is designed for older adults with limited access to other important services that traditional healthcare does not provide.

TRU PACE Philosophy

TRU PACE provides a person-centered approach to care. Our philosophy is based on balancing the medical, emotional, physical and social needs of older adults in our program.

This holistic or All-inclusive plan of care will not only identify what we believe the participant needs but also what they may want.

The Interdisciplinary Team (IDT) customizes each participant’s plan of care to reflect his or her personal needs. We include the participant and family members in the development and approval of the plan of care.  The IDT is made up of;

  • Primary Care Physician: Oversees your entire health care plan.
  • Social Worker:  Assist with everything from finding community resources to emotional support.
  • Physical and Occupational Therapists: Help you stay active, meeting personal physical goals.
  • Dietitian:  Makes sure your meals and dietary needs are met.
  • Home Care Nurse:  Coordinate the supportive services in your home.
  • Clinic Nurse:  Work closely with you and your physician to provide the medical care you need.
  • Transportation Coordinator:  Schedules your transportation to our day center and medical transportation.
  • Personal Care Attendant: Assists with activities in the Clinic and Day Room.
  • Center Director:  Works closely with you and your caregiver to ensure that you receive the resources you need from our program.
  • Recreational Therapist:  Help guide you through fun activities and programs.

TRU PACE participants must receive all needed healthcare, including primary care and special physician services (other than emergency services) from TRU PACE contracted providers. TRU PACE participants may be fully and personally liable for costs of any unauthorized out-of-network services.

History of PACE

PACE began in the early 1970s by On Lok Senior Health Services in San Francisco, CA in order to enable the Chinese community to care for elders without placing them in nursing facilities. With the success of On Lok and subsequent replication projects, Congress authorized PACE as a permanent Medicare provider and Medicaid state option in the Balanced Budget Act of 1997.

TRU Leadership

TRU PACE is a program of TRU Community Care, a Colorado-licensed, Medicare and Medicaid-certified, nonprofit health care organization serving Boulder, Broomfield, Adams, Jefferson, Arapahoe, Denver, and Weld Counties and beyond. In addition to PACE, TRU’s programs include TRU Hospice, TRU Palliative Care, Landmark Memory Care, and TRU Grief Services.

About TRU PACE

TRU PACE (Program of All-Inclusive Care for the Elderly) is a managed health care plan exclusively for older adults. TRU PACE participants receive primary medical care, medication management, physical therapy, specialty services, in-home care, social work assistance, and transportation to and from the TRU PACE day center. With TRU PACE, you can remain living in the comfort of your home and get the medical care you need in a safe environment.
