TRU PACE Medical Care Program for Seniors
TRU PACE provides senior medical, social and homecare services in Boulder, Broomfield, and portions of Adams, Weld and Jefferson counties. Our number one goal is to empower our aging population with the choice to live safely in the home they love for as long as possible. Approved by Centers for Medicare and Medicaid Services, our program is based on a model of care known as PACE (Program of All Inclusive Care for the Elderly). PACE programs keep older adults as healthy as possible in the community by providing comprehensive health care and social services including: primary and specialty medical care, a day health program, social work services, rehabilitation, pharmaceuticals, transportation, home health, and more.
TRU PACE offers senior healthcare solutions. We use Medicare, Medicaid, and private funds to cover all your medically necessary care and services. This eases the burden and worry for you of what is covered and how much you may have to pay. We serve participants 55 and older, by providing preventive and long-term care services for those requiring nursing home level of care in their home and community.

What is TRU PACE? The Program of All-Inclusive Care for the Elderly (PACE), is a Medicare not for profit, Medicaid State option program. TRU PACE is a nonprofit providing managed medical plans for seniors, ensuring that their healthcare needs are met. We are a medical, social and homecare program exclusively for seniors.
Our goal is to provide senior community members with the care, medical treatment and support they need to achieve the highest quality of life – while staying independent as long as possible. Our senior care experts offer an alternative to nursing home placement that features comprehensive, coordinated care for a senior’s medical, social and physical needs, while also providing peace of mind for family caregivers.
The PACE model of care is a home and community based benefit plan, paid for by Medicare, Medicaid, Private Funds, or any combination of the three.

TRU PACE Senior Medical Care
Discover our services, tour our senior care medical clinic, day center, and meet our care team. Our staff includes primary care physicians, social workers, physical therapists, nurses, and more. Many of TRU PACE participants receive most of their care from our expert staff at our center. To make sure you can attend our day health center, we provide all medically necessary transportation to our center for activities and medical appointments.
TRU PACE is designed to meet your individual needs. TRU PACE provides care and services in the home, the community and at our day health center. Seniors are picked up and brought to our Day Center to attend a medical appointment, engage in fun activities and socialization with other seniors. Breakfast, lunch and snack is provided daily for each participant. Activities include fitness and movement programs, art, games, as well as a mixture of special events.
